Care Coordination Program

Coordinated Assistance

Care Coordination is a coordinated, community-based home care program. Services are available to persons 60+ years of age who are homebound, unable to care for themselves, and/or recently discharged from area hospitals who require temporary in-home care.

Independent Living

Care Coordination promotes independent living for senior citizens with supplemental services including assistance minor home repair / modification, one time medication assistance, dental, vision and hearing assistance, and home delivered meals.

Evidence-Based Dental Program

The Texas Department of Aging & Disability Services approved TCOG’s AAA Dental Program as their first evidence-based Dental Program for the aging network in the state in May 2012. Other Area Agencies on Aging will be looking at taking on this program in their regions. This is what promises to be a widely beneficial program, not only for the community of Texoma, but others in the nation as well.

Affordable Professional Dental Care

TCOG AAA Care Coordination program is proactively involved in an innovative approach to provide a much needed affordable dental care program “Smiles For Texoma” (SFT) that assists the continually escalating dental needs of the low-income disabled, frail and elderly citizens of the Texoma area.

Through SFT, participating Texoma American Dental Society members agree to offer treatment at a reduction in normal rates, with the exception of lab fees, which are charged in full. Our goal is to help those seniors who would otherwise have no means of affording professional dental care.

Moving Forward

2016 has brought over $200,000 in grant funding to the Care Coordination program through which 100% of the funds are spent on direct services to Cooke, Fannin and Grayson Counties.
